If you are a self-funded or private patient, you along with your doctor will need to complete a referral form which can be downloaded here. Once the form is complete, email it to us at healthcare@boc.com. Community Funded Patients

BOC supplies a range of HMOT products to community homecare package providers. If you have a homecare package, contact your package provider to ask about sourcing HMOT with BOC. BOC will work with your package provider to get you the equipment you need. Your package provider will work with us to open an account on your behalf.

Government Funded Patients

If the state or federal government is funding your oxygen therapy, they will provide your with the process and what will be provided and the requirements you will need to follow. They will engage BOC to supply the equipment on their behalf and advise you on what is required for ongoing replacement cylinders and or additional equipment.

Veterans

The Department of Veteran’s Affairs funds a range of HMOT equipment for veterans – if you are a veteran, complete this form with your doctor and email it to us at healthcare@boc.com. If you are an eligible DVA cardholder your doctor can also organize the necessary items required for you.

Once I have an account with BOC, how will I receive my HMOT equipment?

As part of our home oxygen therapy service, BOC will have a trained specialist visit you in your home to set up your equipment to your prescribed flowrate and provide the necessary education so you are comfortable with the set up and operation of your equipment.

Who will pay for my HMOT equipment?

Private patients: As a private patient you will be responsible to pay for your own HMOT equipment.

Government funded patient or Veteran: If the DVA or other government organisation is funding your therapy, they will pay for the items covered under their programs and pay on your behalf for those. Some items may not be included – contact your package provider to find out what items you are entitled to.

Community funded patient: If you are receiving a home care package, the cost of HMOT equipment will be charged to your package provider. Some items may not be included – contact your package support provider to find out what items they can assist you with.
